CHRONIC CARE MANAGEMENT
What is Chronic Care Management?
Chronic Care Management (CCM) is a Medicare-supported program designed to help individuals with multiple chronic conditions better manage their health. The program provides ongoing care coordination, improving health outcomes and reducing the need for emergency care or hospitalizations.
Chronic Care Management involves coordinated care for patients with two or more chronic conditions expected to last at least 12 months (or until the patient’s death) and that place the patient at significant risk of decline or complications. Conditions can include diabetes, hypertension, heart disease, asthma, arthritis, and more.
What Does Chronic Care Management Cover?
CCM typically covers:
- Care Coordination: Ensures communication between healthcare providers, specialists, and other services.
- Creation of a Comprehensive Care Plan: Outlines the patient’s medical conditions, goals, medications, and treatments.
- Medication Management: Helps patients understand and adhere to their prescribed medications.
- 24/7 Access to Care: Provides access to healthcare professionals who can address urgent needs.
- Regular Follow-Up: Includes monthly check-ins to monitor and adjust care plans as needed.
- Referrals: Coordinates referrals to specialists or community services.
What Does It Entail?
- Enrollment: Patients must give consent to participate in the program, typically during a conversation with their primary care physician.
- Monthly Services: Patients receive at least 20 minutes of non-face-to-face care each month, often via phone or electronic communication.
- Team-Based Approach: Care is provided by a team, which may include doctors, nurses, and care coordinators.
- Comprehensive Care Plan: As your primary care physician understands your conditions and unique situations, they will create, revise, and monitor a personalized care plan. This plan will be based on your physical, mental, cognitive, psychosocial, functional, and environmental capabilities, while also taking into account resources and support on-hand.
Who Can Help?
- Primary Care Providers (PCPs) are the central point for CCM. They develop care plans and coordinate services within the care team.
- Specialists may collaborate with the PCP to address specific conditions.
- Nurses and Care Managers often handle day-to-day coordination, patient education, and follow-up.
- Pharmacists assist with medication management and adherence.

For New Patients and Existing Patients here are some reminders for your scheduled appointments.
New Patients
- Please arrive early to complete any forms that may be needed.
- Bring your Medicare Card and another form of ID.
- Collect all your medications in a clear bag and bring them to your visits. Also include supplements and over-the-counter medications.
- You may write down their names, dosage, frequency, and what time you take them.
- If you already have your history of health documented, please bring that paperwork. This could include allergies, hospitalizations, illness, etc.
- If you have any other medical information and/or test results such as x-rays or lab work, please bring that as well.
Scheduled Appointments
- Arrive 10 minutes early for your visits.
- Complete the MD Visit Prep prior to your visit.
Appointment Challenges
- Canceling an Appointment If you are unable to keep your appointment or you are going to be late, please call our office as soon as possible. This courtesy allows us to be of service to other patients.
- Late to Appointment Policy We value your time and strive to see you as close to your appointment time as possible. Recognizing unanticipated things happen, we will hold your appointment for a 15 minute grace period after the scheduled start time of your appointment. All appointments will be automatically canceled thereafter. If you arrive after your appointment has been canceled, you will be asked to reschedule the appointment. If you are willing to wait, we will try to provide you with a full appointment. Priority is given to our scheduled patients, which means you may have to wait for an extended period of time. If this is not convenient for you, you may choose to reschedule.
- Missed Appointment or “No-Show” Policy After three (3) missed appointments, we may choose to discontinue your care.
After Hours and Emergencies

In an Emergency or life-threatening situation call 911.

If you need medical care after hours, during holidays, or weekends call our office before going to an Urgent Care. We will treat or direct you to the most appropriate care.
